How to Cancel SSI Payments
- 1). Download and print Social Security Administration Form 521 (Request for Withdrawal Application). The form allows someone who has applied for Supplemental Security Income to cancel that request.
- 2). Fill out the form, making sure all information is correct. Make sure to write legibly and fill out all the fields. Information such as name, Social Security number, application date and benefit type are required. You must also provide a reason for a withdrawal of benefits and a signature.
- 3). Return the completed form to the Social Security Administration. The completed for should be sent to the office nearest your home address. That location can be found using the Social Security Administration local office search online (see Resources for link) or by calling the Social Security hotline at 800-772-1213.
- 4). Wait for a response that confirms a cancellation of your benefits from the Social Security Administration. The agency does not issue a timeline for a response, but you do have 60 days to change your mind if you wish to continue receiving benefits.
- 1). Visit the Social Security Online website, located in the Resources section, and request a password. A temporary password will be mailed to the address on record.
- 2). Wait for the password to arrive in the mail. This method of cancellation works only for people receiving money via direct deposit.
- 3). Visit the Social Security Online website and log in using your Social Security number and the password that was mailed to you.
- 4). Follow on-screen prompts to cancel your SSI direct deposit. The Social Security Administration warns that all payments will be in the form of direct deposit after March 1, 2013, and active accounts should not be canceled.
