How to Fill Out DA Form 2166-8-1
- 1). Utilize the responsibility and accountability section of the NCO Counseling form to review and set goals for the NCO's performance as it relates to his efforts to take care of equipment, work ethic and general sense of responsibility.
- 2). The physical fitness and military bearing component of the form should be completed to reflect the NCO's combat readiness. Physical fitness elements should be graded to include endurance, speed, agility, etc. Military-bearing factors needing to be graded are appearance and professional mannerisms.
- 3). In order to meet the requirements of the leadership reporting section, you will need to grade the NCO on his leadership attributes, such as the ability to influence others.
- 4). As part of the report pertaining to training qualifications, make sure to comment and set goals for the NCO as it regards his ability to contribute to the unit's training regiment.
- 5). In order to detail the NCO's achievements and future goals concerning the values requirement, it will be important to describe his ability to fulfill obligations and review the manner in which he treats others.
- 6). Finally, grade the NCO on competence. Does the NCO demonstrate the necessary abilities to be good at his assignments?
- 7). The NCO Counseling/Checklist Records also requires you to consult with the NCO on his duty description. The duty description plays an important role as part of the review process because it is with this mechanism that the NCO is told of his specific expectations. There are five principle elements to the duties description. These five elements include the principle duty title, duty MOS code, daily duties and their scope, area of special emphasis and appointed duties that are not normally associated with the NCO's duty description.
