You should also check if the company offers reimbursement for treatments done by a doctor or hospital not in their network.

Also write down what steps you can take to appeal a denied claim. Before these though you have to write down other important information too. If you have undergone a medical procedure that needs pre-approval without you filing for approval, chances are, your claim will be denied unless you have a very good reason.

The information file should have information whether the policy covers treatment of mental and emotional illnesses. Then, also put down the individual yearly deductible or the family deductible if that's what you took out. Another data that should be on the information file is the yearly and lifetime maximums.

The information file should have your insurance company identification number. It is easier to write it down than trying to memorize it anyway. Your information file should first have the name, mailing address, and phone number of the insurer. Obviously, this is important especially if you are forgetful.

Another thing that should be there is what medical procedure needs pre-approval from the insurance company. It should also contain what preventive care, if any, is covered by the insurance. This should help you in picking out which preventive care that you have undergone you can file a claim for.
