Most companies require authorization to release personal information, so this will be necessary if there is no formal process in place. In the letter state the reasons you are authorizing the release and also state whether you intend this release to be applicable to future requests or if the release is only granted for this one transaction. Finally, be sure to state to whom you authorize the records to be released.

    • 3). Determine which individual within your company's human resources department is in possession of your driving records and has the authority to release those records. Obtain this individual's contact information and notify him that he may be contacted regarding your driving record.

    • 4). Contact the company that requires your driving record. Give the contact information for the individual within your company who will be providing them with a copy of your driving record. Tell them to contact this individual to request a copy of your driving record.

    • 5). Once the request has been made, check with your human resources contact person to see whether you will be required to pay copying fees and make sure that the record request made by the third party company is being fulfilled.
