    • 1). Go to the Wisconsin Department of Revenue's website. Click on the link for "Businesses," or the direct link for "Business Tax Online Registration" if you are filling out the form online.

    • 2). Gather the information you need for your application including your Federal Employer Identification Number, if you have one; your Social Security Number if you are a sole proprietor; a specific address for your business (not a post office box); a list of all partners or corporate officers and contact information (if applicable) including their Social Security numbers or FEIN numbers; the name, address and account information for your business checking account.

    • 3). Click on the link that says "Application" for Business Tax Registration and download the form. If you want to complete the form online, answer the questions in the boxes provided.

    • 4). Mail your paper application into the Wisconsin Department of Revenue at the address provided on the form. Enclose the $20 registration fee.

    • 5). Wait up to 15 business days for the processing and mailing of your registration certificate. The state indicates that it can take up to 15 days regardless of whether you file online or mail the application.
