    • 1). File a Petition and Order for Name Change with the local court. Include an affidavit or certification with your original name. Visit the county clerk's office or Web site for the necessary forms.

    • 2). Purchase multiple certified copies of the order signed by the judge. A certified copy of the name change order should be available from the court clerk's office.

    • 3). Bring current identification and a certified copy of your Name Change Order to the Social Security office in order to acquire a new Social Security card. There is no fee associated with changing your name with Social Security.

    • 4). Request a new driver's license or identification card from the department of motor vehicles (DMV). Bring the new Social Security card, current identification card and a copy of your court order.

    • 5). Inform employers of the name change so that your company email address and any company documents are updated. This will allow your employer to correct future paychecks, business cards and other necessary information accordingly.

    • 6). Contact other institutions that have account and membership information individually to let them know of the name change. Once your new driver’s license and Social Security card are received, change your name with other institutions such as banks, the Internal Revenue Service (IRS) and insurance providers.
