How to Get a Replacement Form W-2
- 1). Check with your company's payroll department to check that the forms were mailed and when. Confirm they have your current address. Perhaps payroll can issue a replacement form.
- 2). Contact the IRS if you cannot get your employer to give you a replacement. However, before making the call, get some details from your last pay slip, including your company's Employer Identification Number.
- 3). Download and print Form 4852, if necessary, from IRS.gov. This is a substitute for the W-2, which your employer can perhaps complete for you.
- 4). Keep the IRS apprised of all developments. The IRS is on your side and will do its part so your employer will issue a replacement W-2.
