- 1). Gather the required identifying information. To request a Social Security statement, you must have your legal name, Social Security number, date and place of birth and your mother's maiden name. If to estimate your future benefit including future earnings, you should also provide estimates of your future earnings and retirement age.
- 2). Complete the online form or form SSA-7004 to request your benefit statement form.
- 3). Submit the form online (if using the online form) or mail the form SSA-7004 to:
