How to Get Your Criminal History

• 1). Download the request form off the New York Criminal Justice website.
  
  
• 2). Fill out the form in its entirety.
  
  
• 3). For people living in New York State: Go to L-1's website and find an office location near you. Make an appointment and bring all of your application materials with you. These include your request form, two forms of ID (one of which must have a photo), and $61.75 (as of May 2010) payable by cash, check, or credit card.
  
  
• 4). For people living outside New York State: Call the record review unit at 518-485-7675 or 518-457-9847. Request a card scan application packet. Complete all of the included materials (including having your fingerprints taken). Return the application and $61.75 (payable with check, cash, or credit card) to the following address:

• 5). After your request is in, allow seven to 10 business days for your criminal history to arrive in the mail.
