    • 1). Type "Release Request" at the top of the form. If it's for a certain item or set of rights, be more specific. For example, "Transcript Release Request" for a prospective student to sign so that you can access a copy of his transcripts.

    • 2). Enter the text that specifies what this person is releasing. Write in clear terms that by signing this release form, the party is giving you express permission to use or access the item he owns. Include a space in the text where the person can write in his name. For example, "____________ gives ABC Rental Company permission to access a full credit report in his name so that s/he can be considered for a rental unit." The form can also state that the signer releases you from responsibility in case of an issue that arises as a result of the arrangement. Specify in detail the obligations of both the other party and your organization per this release form. If you're giving the person money in exchange for signing this release, explain that arrangement in this paragraph.

    • 3). List the words "Printed Name," "Signed" and "Date" on separate lines after the release statement. Add a long underscored line where the person can fill in his information next to each word. Be sure to provide a duplicate section after that where the party's parent or guardian can sign if any of your contacts are under legal age (17 or younger). The form is invalid if signed by an underage person without consent from an adult.

    • 4). Provide information about your organization, including the logo (if applicable), business name, contact name, address and phone number, centered at the bottom of the release request form.
