    • 1). Type your personal credit application form using a word processing program such as Microsoft Word. If you have a company logo, consider inserting it into the document. There is an example of a personal credit application in the Resources section near the end of this article.

    • 2). Include at the top of the form blank spaces for your customer's identifying information as follows: full name, including a middle name, current address, past address(es), Social Security Number, birth date, home telephone number and work telephone number.

    • 3). Type more blanks for additional essential application information as follows: applicant's employer, how long the applicant has worked there, the name and number of the supervisor, and how much income the applicant makes in a month.

    • 4). Include requests for banking information. Ask what bank the potential borrower uses and how long the account has been open. Depending on what the credit application is for, ask for the balances in their savings and checking accounts.

    • 5). Type more blanks for reference information. You should get your customer to list at least three references, or more if you are selling them a car. Request personal, family, and business references.

    • 6). Include a line for the applicant's printed name and signature.
