    Personal Information

    • The employee must fill out Section One, which requests identifying information about the employee including the employee's name, address, birthday and Social Security Number. The employee must also specify her right to work in the United States as either a citizen, noncitizen national, lawful permanent resident or alien authorized to work.

    Required Documentation

    • The employee must submit documents that prove his identity and his employment authorization. If you have a U.S. passport or permanent resident card, you can prove both with that single document. To prove your identity, you can use a government ID, driver's license, school ID (if it has a photograph), voter registration card, military ID or Native American tribal document. If you are under 18, a school report card, hospital report or nursery school record is acceptable. To prove your right to work, you can use your Social Security card, birth certificate, U.S. citizen ID card or Department of Homeland Security authorization document.
      The employer is responsible for using the documents provided by the employee to fill out Section Two. The employer should list the document title and number, issuing authority and expiration date, if applicable, of the document or documents used. Employers are not allowed to choose which documents they will or will not accept from the list.

    Updating the Form

    • If the employee's name changes, identification expires or she is rehired within three years, the company should use Section Three to update the I-9 form. For name changes, only block A is required to be filled out. For rehires, complete block B. If a person's documentation for her right to work has expired, update block C with the new documentation. The updated document must prove her right to work. Employers must complete the update before the prior documentation expires.
