How to Acknowledge the Source of a Questionnaire

    • 1). List the author of the questionnaire's name if you are referencing the document or using it in it's entirety. Make a "References" heading at the bottom left side of the page. Add the author's last name, followed by a comma and first initial, all in bold print. For instance, "Johnson, J."

    • 2). Note the year the questionnaire was published in regular print and in parentheses after the author's name, then a period, such as (2007). Follow with the questionnaire title -- in regular print -- then a period. Include the publication from which you obtained the questionnaire in italics with a comma at the end, followed by the page numbers on which it appeared in regular print.

    • 3). Provide a byline. Add the author's name beneath the title of the questionnaire if you are using the complete document. For instance, "Developed by John Smith," would be printed below the title in the font and style of your choice. Include the author again in a reference at the end of the document as described in steps 1 and 2.

    • 4). Note the link to the questionnaire if you obtained it from a website. Include this information at the bottom of the questionnaire if you are using the entire document. Place the source reference to the left of the page, put the words "Retrieved from" and a colon in front of the link, then place in parentheses. For instance, if the questionnaire originated from an education website, you would include the link, (Retrieved from: http://www.TeachingTools.com/questionnaire.pdf).

    • 5). Reference the website where the questionnaire was found if you are mentioning it, but not including the entire document. Note this in your references list as described in steps 1 and 2. In lieu of page numbers, put the note, "Retrieved from:" and list the website in regular print.
